CUF Monitoring Report - Overview

  • The CUF Monitoring Report

  • The Report Form must be opened in Adobe Acrobat in order to be completed
    • The Report Form has features such as drop down menus

  • Each section has a box in the upper right corner than can be checked for additional guidance on how to complete that section

  • The Report must be saved to SiteManager if it is a NYSDOT contract. For Subrecipients, the Report can be saved as a PDF or it can be printed.

  • The naming protocol for the Report is
    • Contract # - Region - DBE firm name
    • Example: D123456 - Region 99 - WLDB Training, LLC
