CUF Monitoring Report - DBE Firm's Information - Work Section

Core Work Code: Enter as many codes as the DBE is performing for the DBE Work Category being reviewed. The three digit root number denoting the standard specification section to which the contract pay item relates.
Briefly describe observations of Work / Materials / Equipment: Enter a 1-2 sentence summary of what the DBE is doing in relation to the Core Work Code. To describe associated Work/Materials/Equipment, use the following definitions: Work - the following definitions: Work - the furnishing of all labor, materials, equipment and other incidentals necessary to complete the contract. Work Example: DBE installed bridge bearings. Materials - physical object portion of the work. Equipment - any/all machinery necessary for acceptable completion of work. Materials Example: DBE provided detour signs. Equipment Example: DBE rented attenuator trucks.
Required Attachments: Select most recent approval date for
each document: The AAP20 (Utilization Worksheet is always required). The
Trucking Plan is required for trucking services. The Supplier Questionnaire
is required for materials supply.
AAP-20: The specific contract pay items that a DBE is performing
on a Federally-assisted construction contract. It is also known as a form
AAP 20.
Trucking Plan: The form that lists: the number of trucks being provided by the DBE; whether the trucks are owned or leased by the DBE; and, the back-up calculations of the value of the trucking services. It is also known as a form AAP 23.
Supplier Questionnaire: The form that lists what materials the DBE is providing for which contract pay items. The form includes details such as well as who ordered the materials, if the materials are stockpiled or special-ordered, who arranged for delivery, etc. It is also known as a form AAP 22.
