Reporting of Workplace Violence Incidents

• What is your agency/location process for an employee to file a workplace violence incident report with the agency/location
• Who is your agency/location contact person

All incidents of workplace violence must be reported to a supervisor and the Bureau of Human Resources Management or BASS, in writing, on the DOS Workplace Violence Incident Report.   Reports should be prepared immediately after the incident, noting details that may be forgotten with the passage of time.  Incident reports may be submitted by the victim, employees, supervisory staff or witnesses and will serve as a basis for management to determine an appropriate response.
